Preparing for Your First Visit
New Patient forms to complete before your first appointment
We have our patients fill out paperwork to help Dr. Boeyink and Dr. Wood better assist you with your dental care needs and immediate concerns. If you scheduled an appointment for a patient who is 18 years or older, fill out the New Adult Patient Forms. If you scheduled an appointment for a patient who is younger than 18 years old, fill out the New Child Patient Forms. Please bring these forms with you to your first appointment.
What to expect at your first appointment
For most new patients, the first visit is a checkup. On average, first visits take about 90 minutes, so please plan accordingly. You’ll first see our hygienist for a thorough exam and cleaning where we will take X-rays, intraoral photos, and screen for gum disease and oral cancer. Then, Dr. Boeyink or Dr. Wood will meet with you to discuss our team’s findings, focusing on areas that need attention and suggestions for treatment. Feel free to ask as many questions as you would like - we are here to help you every step of your dental journey.
Bonus: you will be sent home with a toothbrush, travel toothpaste, and floss as a thank you from us!
The first visit to any new health care provider can be a bit overwhelming. Rest assured that our office has a no-pressure atmosphere with a friendly staff who is happy to coordinate your treatment on a schedule you feel comfortable with. No need for rushed treatment, we give you all treatment options for you to take home and consider before making any decisions.
